Identification

We request you bring in both a photo identification and your proof of dental insurance.  For those patients having secondary dental insurance please bring in that proof of insurance also. 

Payments

Payments and insurance co-pays are due at the time services are rendered.

Minors

Minors must be acompanied by a parent or guardian who has the authority to authorize treatment for the minor.

Pre-Medications

Patients who take pre-medications for dental surgery must be accompanied by an adult for post operative instructions and to ensure they arrive at home safely. 

Cancellation / No Show Fees

Appointment cancellations of less than 24 hours and patients who do not show for their appointments will be charged $50.
